Completing this form helps us keep junior members safe, informed, and properly supported while taking part in our activities.
I/We agree to him/her taking part in activities including training away from the club’s base on
pavements/roads in and around Wymondham and the local area, road running, cross
country/trial running, track running, other organised activities as appropriate to WAC training
sessions.(Required)
I/We agree to him/her being photographed for media or group purposes(Required)
I/We give consent to emergency first aid if required(Required)
I am aware that any concerns about my/our child may be shared with relevant agencies(Required)
I have read the Athletics Welfare Policy and Procedures here(Required)
The organisers may ask for further details with regards to some medical needs or behaviour management. This is to ensure all children/young people and adults in the group can enjoy activities as safely as possible
We want you to be safe whilst taking part in activities during sessions and have decided that a code of behaviour may help you think about safety. At all times, either during WAC sessions or elsewhere we expect out junior members to abide by the following behaviours:
Treat everyone how I would like to be treated
No name calling or bullying
Be a good sports person
Have a good attitude, be friendly
Support each other
Listen to instructions and follow any rules
Arrive on time for activities with the appropriate clothes and equipment for the conditions
Ensure you communicate with your coach if you have any concerns/needs during the sessions
Always inform the Club Safeguarding Officer of any concerns/needs you have
To make sure everyone understands these points we would like you to read them with
your parents/carers and ask both of you to sign the form below. Most of all we hope
you enjoy running at Wymondham AC!
